What happens when you stop semaglutide?

Finishing my 6-month run of semaglutide (lost 35 lbs) and my doctor and I are planning to taper off. But everything I read says people gain the weight back. What's the actual data and has anyone successfully maintained weight loss after stopping?

The data is sobering. The STEP 4 trial found that patients who discontinued semaglutide regained about 2/3 of their lost weight within a year. The STEP 1 extension data showed similar trends. This isn't unique to semaglutide — it's true of essentially all weight loss interventions. The body's set point mechanisms are powerful. However, this doesn't mean it's hopeless. Patients who maintain lifestyle changes (high protein diet, regular resistance training, behavioral strategies) tend to regain less. Some research suggests a gradual taper rather than abrupt discontinuation may help.

I'm terrified of this. Part of why I've been so focused on building sustainable habits while the appetite suppression makes it easier. If the medication gives you a window to establish new eating patterns, exercise habits, and metabolic improvements, the hope is those habits carry forward even when the drug effect fades.

Some patients transition to a lower maintenance dose rather than stopping completely. Others cycle on and off. There's no one-size-fits-all answer, but having a clear plan with your provider before stopping is essential. The worst outcome is stopping cold with no strategy.
