PT-141 timing and dosing — what worked for you?

Trying PT-141 (bremelanotide) for the first time. My doc prescribed it but the guidance on timing was vague — just "take it before sexual activity." How far in advance? I've read anywhere from 1 to 4 hours. And is the 1.75mg standard dose about right or do some people need to adjust? Also want to hear about side effects. I know nausea is common. Anything else to watch for?

The FDA-approved dosing for bremelanotide (Vyleesi) is 1.75mg subcutaneous, taken at least 45 minutes before anticipated sexual activity. In practice, most people find 2-3 hours beforehand works best for peak effect. It works through melanocortin receptors in the brain (MC4R primarily), so it needs time to cross the blood-brain barrier and activate the pathway. Nausea is the most common side effect (about 40% in clinical trials) and tends to be worst the first 1-2 uses, then diminishes. Taking it on a light stomach helps. Some people also report facial flushing and a slight increase in blood pressure — monitor if you have hypertension. Don't use it more than once in 24 hours or more than 8 times per month per FDA guidance.

I'll share a female perspective since PT-141 is actually FDA-approved for women (premenopausal, for HSDD). The 2-hour timing worked for me. Nausea was real the first time — had some ginger tea ready which helped. By the third use the nausea was basically gone. The effect itself is distinctly different from anything psychological — it's a genuine physical arousal response. Effective but I use it sparingly, not as a daily thing.
